Q: What insurance carriers do you accept?
A: We accept most major insurance carriers. However, it is the patients’ responsibility to determine coverage. Check with your insurance plan to determine benefits prior to your appointment.

Q: Where are you located?
A: Our main office is located at 3500 Executive Parkway in Toledo; we also have offices in Fremont, Archbold, Bowling Green, and Maumee. Please see the locations section of this website for maps and addresses of all our locations.

Q: What do I need to bring to my visit?
A: Please bring your current insurance card, a photo I.D., method to pay for your co-pay and/or balance due, a list of current medications (including dosages), and any images, notes or test results relevant to your current health concern. If this is your first visit please complete the new patient forms that can be printed off from this website and bring them to your visit.

Q: What forms of payment do you accept?
A:  We accept cash, checks and credit cards. The credit cards we accept are: MasterCard, Visa and Discover. You may also use a debit card to pay for services. Please remember co-pays and any unpaid balances are due the day of your appointment.

Q: How long will my first appointment take?
A: Your initial consult may take anywhere from 15 to 45 minutes depending on your urologic concern. Occasionally, one of our physicians may be delayed due to an emergency, we appreciate your consideration.

Q: Do I need a referral from my primary care doctor to visit a urologist?
A: Please check with your insurance company to determine whether a referral is required.

Q: How do I schedule an appointment?
A: Appointments can be made by filling out a request form on this website. Additionally, you may make an appointment via telephone, a list of all our locations and telephone numbers are listed on this site.  Please have your insurance information available when making an appointment.
